Requests & Forms

Work Orders should be filled out for repairs to existing facilities, furnishings, and equipment. (Maintenance, Grounds, Electro-Mechanical, Housekeeping, etc.)

Modification Requests will change or add to the structure of a building. After submitting the request, Physical Plant will then provide a cost estimate for the modification. If the estimate is approved by all the necessary individuals and funding is made available by the Department, Physical Plant will then schedule the work requested.

Vehicle Requests are available to University employees for business purposes. The University has a limited supply of vehicles and such vehicles are assigned on a first-come, first-served basis.
